Strategies For Establishing Private Virtual Hospitals Revealed

Identifying barriers and enablers can lead to effective implementation and sustainability of virtual healthcare models.

The rise of virtual healthcare has transformed how medical services are delivered, especially during the COVID-19 pandemic when remote care became more prevalent. A recent study highlights the implementation strategies necessary for establishing a private virtual hospital in Australia, aiming to improve suitability, acceptability, and uptake of such innovative healthcare models.

Research conducted by Fisher and colleagues identified several key determinants influencing the successful implementation of virtual hospitals. Conducted between February and July 2023, the qualitative study involved 37 interviews with various stakeholders, including consumers, clinicians, and health service leaders. The findings reveal both enablers and barriers to the establishment of private virtual hospitals.

Strong executive leadership, commitment to high-quality care, and enthusiasm for extending services to rural and remote communities emerged as significant enablers. Participants emphasized the necessity of having clear visions and organizational readiness for change, which can facilitate the transition from traditional healthcare models to virtual ones. Leaders expressed confidence about the future of digitally enabled healthcare, appreciating its potential to improve access for patients.

Despite this optimism, the study identified major barriers such as restrictive funding models. These financial constraints inhibit innovation by limiting the reimbursement for virtual care services. Current Medicare funding regulations do not support consultations conducted via telehealth for admitted patients, presenting significant challenges for virtual hospital implementation.

Other challenges mentioned included technological limitations, workforce issues, and varied acceptance of telehealth practices among clinicians. Agreement on the suitability and safety of virtual healthcare varied significantly, showing disparities based on individual experiences and comfort levels with technology.

Consumer respondents voiced strong preferences for face-to-face consultations, expressing concerns about the risks of virtual assessments leading to misdiagnoses. Nonetheless, many acknowledged improvements have been made since the onset of the pandemic, affecting their perceptions of virtual care.

Participants highlighted the importance of seamless communication systems to facilitate effective virtual healthcare delivery. Ensuring stakeholders have reliable access to digital platforms is emphasized as inadequate technological integrations may lead to poor patient experiences.

Fisher and the research team employed the Planning and EvaluATING Remote Consultation Services (PERCS) implementation framework to guide their analysis. The findings from this framework not only informed recommendations for the establishment of the private hospital but also provided insights applicable to similar efforts globally.

The researchers also found the organizational readiness for change among staff to be adequate, but expressed concerns about training needs and workforce retention. Participants articulated the necessity for supporting clinicians transitioning to virtual healthcare delivery by establishing training programs and resources.

Overall, the study calls for urgent policy changes to address the existing funding barriers, advocating for the amendment of legislation affecting telehealth reimbursement for admitted patients. This could catalyze the establishment of more private virtual hospitals across Australia and beyond, unlocking the full potential of remote healthcare models.
