Mpox Outbreak Spreads Urgent Calls For Health Response

Countries across Africa mobilize vaccination campaigns amid rising Mpox cases

The outbreak of Mpox, formerly known as monkeypox, is causing significant concern across Africa, particularly within the Democratic Republic of the Congo (DRC) where the epidemic has reached alarming levels. The specific strain of the virus, Clade Ib, has evolved, showing greater transmissibility, and has compelled health authorities to take swift action.

Recently, the Africa Centers for Disease Control and Prevention (Africa CDC) recognized the DRC's efforts in launching its vaccination campaign targeting the hardest-hit regions. This initiative kicked off with the distribution of vaccines to vulnerable populations across Equateur and North Kivu provinces, marking what health officials deem as a pivotal moment to control the surge of Mpox cases.

Statistics tell the story of the outbreak’s severity: as of early October 2024, the region has seen some 34,297 Mpox cases, including 6,806 confirmed infections and 866 fatalities. The DRC alone accounts for over 80% of these figures, raising alarm among public health experts who fear for the vulnerable groups most affected, particularly children and individuals with weak immune systems.

Health Minister Roger Kamba emphasized the importance of targeting frontline health workers first, who face the highest risk of exposure. The initial vaccine rollout aims to safeguard these individuals before extending the immunization efforts to the general population. UNICEF estimates highlight children under 15 years as the predominant group affected, underscoring the importance of vaccination efforts as they prepare to receive vaccines sufficient for their age group.

The DRC’s proactive stance has garnered international support. The World Health Organization (WHO) recently declared Mpox as a global public health emergency, spurring donations of 265,000 vaccine doses from the European Union and the United States. Health leaders like Africa CDC Director-General Jean Kaseya applauded the aggressive response taken by the DRC to mitigate the outbreak.

Yet, the rise of alternative variants, particularly Clade Ib, raises questions about the efficacy of existing vaccines. Preliminary data from studies indicates historical death rates of up to 10% for this strain, with recent outbreaks showing mortality rates between 1% and 3.3%. Conversely, Clade II, the strain observed during the 2022 outbreak, has reported over 99.9% survival rates, highlighting the need for urgent research and adaptation strategies.

The rapid development and deployment of vaccines is critically important, but logistical challenges remain. Initial doses of Edinburgh's Bavarian Nordic vaccine have been restricted due to age-related approvals, as many available vaccines were not approved for pediatric use. The anticipated arrival of additional doses for children provides hope for broader vaccinations.

At the same time, the health sector faces compounded challenges as millions of vaccine doses are needed to stem the illness’s spread. Experts project the need for around 10 million doses to effectively control the contagion, echoing past monitoring systems within public health frameworks.

Perhaps most pivotal to the response is the need for innovative therapeutic strategies. With vaccine responses not solely targeting the broader population, there is increasing consideration for monoclonal antibodies directly aimed at neutralizing viral infections.

Stefano Gullà, Chief Scientific Officer at Kling Biotherapeutics, discussed how new methodologies for monoclonal antibody research could support broader vaccination efforts. By rapidly isoling effective antibodies from human B-cells, researchers can develop therapies targeted at newer strains as they emerge.

This approach, which allows for rapid scaling of antibody production, could potentially provide immediate therapeutic options for individuals already infected with Mpox. Given the delays often faced with vaccine distribution, alternative protective measures must be expedited, especially for high-risk groups.

Looking beyond the DRC, Rwandan health officials recently received 5,420 doses of the MVA-BN vaccine as part of the European Union’s extensive support program aimed at managing the Mpox outbreak. This follows the DRC receiving around 200,000 vaccine doses, highlighting the urgent cooperative efforts needed among governments to combat this growing health crisis.

The commitment to global health cooperation is seen through joint efforts such as the Pandemic Fund, which allocated significant financial resources to combat health adversities across the continent, promoting surveillance and enhancing lab capabilities.

With cases expected to escalate, primarily from the DRC, there remains uncertainty surrounding this virus’s continued evolution and potential for wider outbreaks. Effective global solidarity is less of a luxury now and more of necessary strength as health systems across the continent unite to tackle Mpox.

While there are hopes pinned on the surge of vaccines and therapeutic strategies, the broader response remains to be seen. The World Health Organization and various health agencies stress the urgency for continued vigilance, immediate responses, and adaptable strategies. Health authorities urge communities to remain aware and engaged as significant efforts ramp up to combat this challenging public health battle.
