On December 13, 2022, Flintoff was testing a Morgan Super 3, a three-wheeled sports car capable of reaching speeds up to 130 mph, when the vehicle flipped over and crashed off the test track. At the time of the accident, Flintoff was not wearing a helmet, which raised concerns about safety protocols on the show. Despite the car being equipped with roll bars designed to protect occupants, Flintoff suffered serious injuries that would take months to recover from.

In a candid interview on The Jonathan Ross Show, Flintoff disclosed that he did not leave his home for approximately six to eight months following the crash, as he struggled to cope with the physical scars and the mental aftermath of the incident. "No secret that I had a car accident filming Top Gear," Flintoff stated. "Afterwards, obviously there’s the physical scars that I’ve got. But then the mental side of it. I didn’t leave the house for probably six or eight months. The only times I was leaving the house was for medical appointments and surgeries."

Flintoff’s recovery journey was not just about healing physically; it also involved confronting the mental health challenges that arose from the accident. He recalled how a close friend, Rob Key, encouraged him to attend cricket matches, albeit from a distance, while he wore a full face mask to cover his injuries. This gradual reintegration into the world helped him find a semblance of normalcy. "I started getting back into it and started to find my feet a little bit. Ever since I can remember, from being a kid, cricket was a massive part of my life," he said.

In September 2023, Flintoff reemerged in the public eye as he began working as an unpaid mentor to the England bowlers. He later took on assistant coaching roles with England’s white-ball teams and was appointed head coach of the England Lions, a position aimed at developing players for the senior team.
