Congo Struggles To Contain Mpox Outbreak As Vaccines Arrive

With thousands of cases reported, health officials ramp up efforts to combat the dangerous virus amid humanitarian crises

The Democratic Republic of Congo (DRC) is facing its most severe mpox outbreak to date, with the latest numbers skyrocketing as health officials scramble to contain the rapidly spreading virus. The situation took a significant turn recently when approximately 99,100 vaccine doses arrived at Kinshasa airport, part of the first shipment from the European Union. These vaccines, flown from Copenhagen, are the result of urgent international cooperation aimed at addressing the public health crisis.

This outbreak, now considered the epicenter of mpox cases globally, has reported over 21,000 confirmed and suspected mpox cases, and more than 650 fatalities across Africa, primarily concentrated within Congo's borders. The World Health Organization (WHO) has categorized this as a global health emergency due to the emergence of the new Clade 1b strain, which is fueling the outbreak. The total cases across Africa have topped 25,093, indicating the widespread nature of the outbreak, which is also affecting countries like Burundi, Republic of the Congo, and Central African Republic.

Laurent Muschel, head of the EU’s Health Emergency Preparedness and Response Authority (HERA), and Congolese Health Minister Samuel-Roger Kamba were present for the arrival of the first shipment. Muschel praised the swift cooperation between the EU and African nations, reinforcing the theme of solidarity. This first delivery was part of the EU's commitment to send out 560,000 doses to the DRC and other countries impacted by mpox.

Health experts have identified the origins of the current outbreak and its rapid progression, noting the strain's probable origins from wildlife, as mpox is classified as zoonotic, meaning it is transmitted from animals to humans. This may include contact with infected primates or rodents, which are natural reservoirs for the virus. Initial cases also suggested it may spread through close human contact, marking concerns for communities where healthcare access is limited.

The ripple effects of the outbreak extend beyond terms of health. The DRC is grappling with significant systemic challenges, including longstanding conflicts and inadequate healthcare infrastructure. Health services are already overstretched, especially amid simultaneous outbreaks of other diseases such as cholera. This crisis is putting intense pressure on healthcare workers already managing multiple emergencies.

Healthcare facilities, particularly those situated close to conflict zones, are struggling to deliver care effectively. Reports from Dr. Musole Mulambamunva Robert, medical director of the Kavumu hospital, indicate they sometimes treat four times as many patients as their facilities are equipped for. This dire situation often sees patients waiting for hours or even days for treatment.

The chaotic logistics of delivering vaccines to eastern Congo pose another layer of complexity. Amid significant violence and displacement within the region—spurred by over 120 armed groups vying for control of mineral-rich lands—many health clinics are located far from communities. This makes access challenging and dangerous for healthcare workers and patients alike. Mercy Muthee Lake of the International Federation of Red Cross and Red Crescent pointed out the difficulties, stating some communities might require hours of travel along risky routes to reach basic health services.

Given these extraordinary challenges, there is heightened concern around vaccine accessibility. While the initial delivery of vaccines is encouraging, only around 250,000 doses have arrived so far, and officials estimate they need up to 3 million doses to effectively stem the outbreak. Currently, the approved vaccines are intended for adults only due to limited data on the safety and efficacy of the vaccine for children. Concerns about vaccination rates are compounded by barriers such as malnutrition, which can exacerbate health complications, and high rates of undiagnosed HIV among the population.

While the DRC is making strides to combat the mpox outbreak, health authorities face multifaceted challenges, including logistical barriers, political instability, and overt conflict. The government has mobilized part of its healthcare apparatus to step up efforts against this unprecedented outbreak. Becker’s Health reports on continued initiatives to bolster emergency response, including training for local health workers, community outreach programs, and the establishment of treatment centers dedicated to isolations, such as the Munigi mpox treatment center.

International organizations like the WHO are closely monitoring the situation, providing support and guidance to local health authorities. With more vaccine shipments on the horizon and continued advocacy for increased international support, health leaders hope to turn the tide once again against mpox. The collaborative international response showcases the commitment to public health, emphasizing the need for collective action against disease outbreaks, particularly within vulnerable populations.

There’s no denying the road to recovery will be challenging, but the determination of the Congolese health sector, supported by the international community, can serve as the cornerstone to overcoming this crisis.
